Tracking Outcomes as a SW/CRS

When a client enters Make Progress, SW/CRS track their progress in three outcome areas: Spiritual, Vocational, and Housing. Actions taken by Make Progress staff are logged in the system, visually displaying client progress on the Make Progress Program Engagement. This allows staff to know at any moment where the client is in meeting their outcomes.

This article describes how to track outcomes as a SW/CRS.

Spiritual Outcome Area

1. Offered Prayer: When an SW/CRS offers prayer during a Client Session, they log this under Activities Performed when writing their Client Note. To learn how to create a Client Note, click here.

2. Offered Salvation: When an SW/CRS offers salvation during a Client Session, they log this under Activities Performed when writing their Client Note. To learn how to create a Client Note, click here.

3. Offered Baptism: When an SW/CRS offers baptism during a Client Session, they log this under Activities Performed when writing their Client Note. To learn how to create a Client Note, click here.

Vocational Outcome Area

1. 2 Forms of ID: SW/CRS records that the client has 2 forms of ID either during the Vocational Assessment or on the Pre-Employment Checklist in the Make Progress Program Engagement. 2. Vocational Assessment: SW/CRS records a Vocational Assessment. To learn how to complete the Vocational Assessment, click here.

3. Has Working Phone: SW/CRS records that the client has a working phone either during the Vocational Assessment or on the Pre-Employment Checklist in the Make Progress Program Engagement. 4. Childcare: If the client is a mother, SW/CRS records that the client Has CAPS either during the Vocational Assessment or on the Pre-Employment Checklist in the Make Progress Program Engagement. 5. Resume: SW/CRS records that the client has a recent resume during the Vocational Assessment. To learn how to complete the Vocational Assessment, click here.

6. Voicemail: SW/CRS records that the client has a working phone with voicemail during the Vocational Assessment. To learn how to complete the Vocational Assessment, click here.

7. Email: SW/CRS records that the client has a simple email with their name during the Vocational Assessment. To learn how to complete the Vocational Assessment, click here.

8. Employment Applications: If a client does not attend Next Steps or secure a job during Job Search but later obtains a job while still at AM, the SW/CRS creates a Staff-Approved Job. This indicates that the client has also created Employment Applications. To learn how to create a Staff-Approved Job, click here.

9. Staff-Approved Job: If a client does not attend Next Steps or secure a job during Job Search but later obtains a job while still at AM, the SW/CRS creates a Staff-Approved Job. To learn how to create a Staff-Approved Job, click here.

10. Vocational Health Goals: Each time an SW/CRS meets with a client, they need to create and update Goals within the Vocational Health Case Plan. To learn how to add a Goal to a Case Plan, click here. To learn how to update the Status of a Goal, click here.

Housing Outcome Area

1. Credit Check: SW/CRS enters client's credit score during the Vocational Assessment. To learn how to complete the Vocational Assessment, click here.

2. Housing Health Goals: Each time an Counselor meets with a client, they need to create and update Goals within the Emotional Health Case Plan. To learn how to add a Goal to a Case Plan, click here. To learn how to update the Status of a Goal, click here.

When the SW/CRS performs one of these Outcome Activities, both the individual Outcome area donut chart and the Outcome listview with Points will update automatically.
