Tracking Outcomes as a Counselor

When a client enters Make Progress, Counselors track their progress in three outcome areas: Spiritual, Emotional, and Relational. Actions taken by Make Progress staff are logged in the system, visually displaying client progress on the Make Progress Program Engagement. This allows staff to know at any moment where the client is in meeting their outcomes.

This article describes how to track outcomes as a Counselor.

Spiritual Outcome Area

1. Offered Prayer: When a Counselor offers prayer during a Client Session, they log this under Activities Performed when writing their SOAP Note. To learn how to create a SOAP Note, click here.

2. Offered Salvation: When a Counselor offers salvation during a Client Session, they log this under Activities Performed when writing their SOAP Note. To learn how to create a SOAP Note, click here.

3. Offered Baptism: When a Counselor offers baptism during a Client Session, they log this under Activities Performed when writing their SOAP Note. To learn how to create a SOAP Note, click here.

Emotional Outcome Area

1. Sessions: Counselors record a SOAP note after each one-on-one session with a client. Note that points differ for Sessions based on persona. To learn how to record a SOAP Note, click here.

2. Step 3: Counselors update the status of the Step 3 Goal in the Emotional Case Plan when it is complete. To learn how to update the status of a Goal, click here.

3. Recovery Groups: Counselors track attendance for the Recovery Groups they facilitate. To learn how to track attendance for a group, click here.

4. DLA Score: Counselors record the DLA assessment monthly. To learn how to record a DLA Assessment, click here.

5. Emotional Health Goals: Each time an Counselor meets with a client, they need to create and update Goals within the Emotional Health Case Plan. To learn how to add a Goal to a Case Plan, click here. To learn how to update the Status of a Goal, click here.

Relational Outcome Areas

1. Identify a Sponsor: Counselors update the status of the Identify a Sponsor Goal in the Emotional Case Plan when it is complete. To learn how to update the status of a Goal, click here.

2. Relational Health Goals: Each time an Counselor meets with a client, they need to create and update Goals within the Relational Health Case Plan. To learn how to add a Goal to a Case Plan, click here. To learn how to update the Status of a Goal, click here.

When the Counselor performs one of these Outcome Activities, both the Outcome donut chart for that Outcome Area and the Outcome listview with Points will update automatically.
