Case Managers: Recommending a Persona Change for a Client

Updated on

As Case Managers continue to get to know clients on their caseloads there may come a point that a change in persona is needed. A client's persona is assessed at screening based upon their highest presenting need at the time. However, as a client stabilizes or becomes more highly motivated, they may share more information and their highest presenting need may change. When this occurs, we need to adjust their persona.

This article describes how to recommend a persona change. To learn how to submit a client to the next Program Phase, click here.

From the Client's Active Program Engagement.

1. Click Recommend Persona Change.

2. Press the toggle by Change Persona.

3. Select the persona you'd like to Update Persona To.

Lim. means Limited and indicates the client should be served in Find Hope for 30 days. Typically, Limited is recommended when a client has low motivation to complete the program, is defiant or resistant, or is expressing that they'd only like to stay for 30 days.
Exp. means Expanded and indicates the client is motivated to complete the program and eligible to receive expanded services in Choose Help and Make Progress.

4. Enter any Persona Update Comments.

5. Press Next.

6. Click Next.

Your leader has been alerted and will review your request to change the client's persona.
