As Counselors in Make Progress continue to meet with clients on their caseloads, we need to regularly perform the DLA to measure a client's mental health and disability impact on daily activities over time. The DLA score also impacts a client's ability to move on to the @Work program.
This article describes how to record a DLA.
Begin from the client's Contact Page.
1. Click Assessments.
2. Scroll down and click New Assessment.
3. Select DLA.
4. Click Next.
Depending on your internet connection, it may take a few moments for the assessment you selected to load. During this time you will see dots on the screen. Keep waiting and it will load.
5. Press Click to go to DLA Assessment.
6. Click Edit.
7. Scroll down to Activity Scores and answer all 20 questions.
Each DLA questions is rated on a score of 1 to 7. 1 is low, 7 is high.
8. Click Save.
The DLA Assessment has been created and can be found in the Assessments section of the Contact as well as the client's Make Progress Program Engagement.
You can view the DLA score over time on the client's Contact Page under Assessments. Change the dropdown to DLA to view.
