Navigating Case Plan Pages

From the Case Plan Page, staff can see all details for the client's case plan and goals for a particular phase or outcome.

This article describes how to navigate the Case Plan page.

When a client first arrives at an Atlanta Mission downtown campus, depending on their track, a Find Hope or Choose Help case plan is automatically created for them. When they move into Make Progress, 5 Case Plans are automatically created based on the Outcomes - Spiritual, Emotional, Vocational, Physical, and Housing. When they move into Sustain and Grow, a Sustain and Grow case plan is automatically created for them.

Highlights Panel

1. Case Plan Name: Always states Case Plan Type - Date Started: Client Name.

2. Client's Name: Links back to the Client's contact page.

3. Owner: What staff member currently is managing this case plan.

4. Total Goals: How many total goals are currently in this case plan.

5. Status: The current status of the case plan indicating whether it has begun, is in progress, abandoned, or completed.

6. Quick Action buttons.

Case Plan Description

7. Description: Describes the purpose of the particular type of Case Plan.

Goal section

8. New Goal button: Used to create a new goal for the client's Case Plan.

9. Minimized Goal View: When the arrow is to the right, you see just the top line items of an individual goal like the Name, the Status, and whether it has action items.

10. Expanded Goal View: When the arrow is pointing down, you see any action items for the goal as well and their status.

An action item is a task to work towards a goal.

Client Note Relationship section

11. Related Individual Client Notes: When a client note has been related to a goal upon the note creation, you'll see it listed in this section and can click through to read the note.

System Information Section

12. Created By: Shows what staff member created the Program Engagement.

13. Contact: Links back to the client's contact page.

14. Last Modified By: Shows what staff member last modified the Program Engagement.
