Recording an Internal AM Referral

As Case Managers in Find Hope and Choose Help at The Shepherd's Inn continue to meet with clients on their caseload, they may discover that a client would be best served at The Potter's House. When this occurs, the client needs an Internal AM Referral and to apply to The Potter's House.

This article describes how to record an Internal AM Referral and how to begin the application to The Potter's House. To learn how to record an External Community Referral, click here.

Begin from the client's active Program Engagement.

1. Click Create Service Deliveries.

2. Select Internal AM Referral.

3. Click the Right Arrow.

4. Click Next.

5. Set the Application Date.

The Application Date is often the same day as the referral, but should the client plan to apply at a later date, add that date here.

6. Enter in the Referral Notes for why you are referring the client.

7. Click Next.

8. Press TPH Application Link.

The TPH Application will automatically load and will prefill the application with the information we already have in the system on the client.

9. Have the client fill out the application.

The Internal AM Referral has been created and the application is in the queue of the TPH Intake Specialist.

The Internal AM Referral service delivery you just created can be found in the Service Deliveries section on the client's Program Engagement.
