Recording a Needs Assessment

As Case Managers in Find Hope and Choose Help  continue to meet with clients on their caseloads, a client may express interest in moving forward to Make Progress. When a client expresses this interest, we need to record a Needs Assessment.

This article describes how to record a Needs Assessment.

Begin from the client's Contact Page.

1. Click Assessments.

2. Scroll down and click New Assessment.

3. Select Needs Assessment.

4. Click Next.

Depending on your answer to each question, more questions may appear.

If a client answered one of these questions during screening or intake, their answer will appear above the related question for reference.

5. Answer the Psychological/Emotional section questions.

6. Press Next.

Some pages will have tables or rubrics to explain a question's rating scale. If you're unsure, read over the image and then select your answer.

7. Answer all the Safety/Crisis Risk section questions.

8. Click Next.

9. Answer all the Substance Use section questions.

10. Click Next.

11. Answer all the Daily Living section questions.

12. Click Next.

13. Answer all the Vocational section questions.

14. Click Next.

15. Answer all the Level of Motivation section questions.

16. Press Next.

17. Indicate your overall recommendation for the client concerning Make Progress.

18. Press Next.

19. If you don't intend to submit the client for approval to Make Progress, press Click here to view the Needs Assessment.

20. Otherwise, select Submit for Submit to Make Progress.

21. Click Next.

If you click to Submit to Make Progress, the Needs Assessment will take you to submit an approval. Click here to learn how to do so.

The Needs Assessment has been created and can be found in the Assessments section of the Contact as well as the client's Find Hope or Choose Help Program Engagement.
