Helping a Client Fill Out the Intake Form and Waivers at Restoration House

When a client has been accepted into Restoration House or My Sister's House, we need them to fill out the intake form in order to collect information important for their caseworker as well as to have them sign Community Living waivers, photo waivers, custody agreements, etc.

This article describes how to help a female client fill out the WACS Intake Form.

1. Load the Intake Form on the Windows Dock on the computers in the computer lab.

2. Instruct the client to fill out all required fields and to have you check the form when they get to the Verification Screen.

Additional questions may appear based on their answer to a previous question.

3. When the client reaches the Verification screen, verify their name and birthdate matches what they shared at Screening.

If a client has entered in name or birthdate slightly differently from what they shared at Screening, the data will not pass into Salesforce. Their name or birthdate should be changed in the General Section and then the form submitted.

If you realize after the fact that the information was incorrect, let the Systems team know and we'll push the data through.

4. If the information is correct, check Intake Specialist has verified this form.

5. Click Submit.

If the client is a mother bringing a child or children with her to Restoration House, she will be taken automatically to the Children's Intake form when she presses submit.
