What it flags:
- Mis-checked boxes (e.g., physical limitations)
- Misspelled names, incomplete phone numbers, incorrect DOB
- Duplicate contacts
- Wrong motivation values
Why it matters:
- Safety risks: approving ineligible or banned clients.
- Wrong track: incorrect persona/track assignment.
- Record integrity: missing/duplicate records that confuse case management and delay care.
- Duplicates = missed history: usually means an existing record was overlooked, along with critical info (mental health, substance use, prior exit reasons, active bans).
Operational impact: Creates avoidable rework for teammates, leaders, and systems staff.
