Understanding my Referral Metrics

The My FH Social Worker Performance dashboard helps you track your performance and meet role expectations. A key part of this is your referral metrics, which track both how often you're referring clients on your caseload to community partners and how often you're meeting with those partners. These metrics reflect your efforts to build and maintain strong relationships with city partners and ensure clients are connected to the resources they need.

This article explains how to read and understand your referral metrics.

The reports described below include all referrals you've made as well as meetings with partners.

Providing Referrals to Clients Prior to Exit

A Referral for exited clients is tracked by creating a Service Delivery of a Referral with a type of External Community Referral.

1. 📨# of Referrals (Last 30 Days, Exited Clients): This tile shows the number of referrals to extrernal community partners that you've made for clients who exited within the last 30 days.

2. 📨# of Referrals (This FY, Exited Clients): This tile shows the number of referrals to extrernal community partners that you've made for clients who exited within this fiscal year.

3. ‼️# Missing Referrals(Last 30 Days, Exited Clients): This tile shows how may clients on your caseload exited Atlanta Mission in the last 30 days without a referral logged in Salesforce.

4. ‼️# Missing Referrals(This FY, Exited Clients): This tile shows how may clients on your caseload exited Atlanta Mission this fiscal year without a referral logged in Salesforce.

5. 🏘️📨 Provide 50% of Clients with Quality Referrals or Housing Upon Exit(Last 30 Days): This tile shows what percentage of exited clients on your caseload had a referral logged in Salesforce within the last 30 days. The expectation is that at least 50% of exited clients from your caseload would have a referral to an external community partner.

6. 🏘️📨 Provide 50% of Clients with Quality Referrals or Housing Upon Exit(This FY): This tile shows what percentage of exited clients on your caseload had a referral logged in Salesforce this fiscal year. The expectation is that at least 50% of exited clients from your caseload would have a referral to an external community partner.

Meeting with Community Partners

Meetings with Community Partners are tracked by staff members creating an Activity with the Type set to "Meeting" on the relevant Referral Account.

7. 🏢Meet with 2 or More Community Partners and Organizations Monthly (Last 30 Days): This tile shows how many Community Partners you met with in the last 30 days.

8. 🏢Meet with 2 or More Community Partners and Organizations Monthly (This FY): This tile shows how many Community Partners you met with this FY, broken up by month.

You get a referral, and you get a referral, and you get a referral!
