Recording an External Community Referral

As Case Managers continue to meet with clients on their caseload, we need to document the referrals we make to outside community partners.

This article describes how to record an External Community Referral. To learn how to record an Internal AM Referral, click here.

Begin from the client's active Program Engagement.

1. Click Create Service Deliveries.

2. Select External Community Referral.

3. Click the Right Arrow.

4. Click Next.

5. Search for and select the Referral Organization.

6. Select the Referral Type.

7. Select the Referral Reason.

8. Enter in the Referral Notes for why you are referring the client.

Make sure to add as much detail under notes as you can.

9. Click Next.

10. Press Click here to view your created records.

10. Click on the External Community Referral you created.

The External Community Referral has been created.

The External Community Referral service delivery you just created can be found in the Service Deliveries section on the client's Program Engagement.
