Tracking Outcomes as a Sustain and Grow Leader

When a client enters Sustain and Grow (SG), SG Leaders track their progress in three outcome areas: Rooted in Community, Retain Employment, and Secure Sustainable Housing. Actions taken by SG Leaders are logged in the system, visually displaying client progress on the Sustain and Grow Program Engagement. This allows staff to know at any moment where the client is in meeting their outcomes.

This article describes how to track outcomes as a SG Leader.

Rooted in Community Outcome Area

1. Attend Regular Community Meetings and Events: SG Leaders track attendance for the Campus Community Meetings and Events. To learn how to track attendance for a class, click here.

2. Engage with 2 External (recovery) Communities Over the Course of 3 Months Minimum: SG Leaders track attendance for Recovery Meetings as a class when Substance track clients bring them letters that indicate they attended. To learn how to track attendance for a class, click here.

3. Walk-through community resource book and determine which communities to try out: SG Leaders update the status of the Walkthrough Community Resource Book and Determine which Communities to Try Out Goal in the Sustain and Grow Case Plan when it is completed by a client on the Substance track. 4. Evidence of One-on-One Meetings with SG Staff: SG Leaders record a Client Note after each one-on-one session with a client. To learn how to record a Client Note, click here.

5. Have an Emergency Contact/Sponsor Outside of Atlanta Mission: SG Leaders either fill out the Emergency Contact fields on the Sustain and Grow Program Engagement or, if the client is on the Substance Track, they update the status of the Obtain Sponsor Goal in the Sustain and Grow Case Plan when it is completed.

Retain Employment Outcome Area

1. Continued Employment for 3+ Months: If a client loses their Staff-Approved Job, SG Leaders end the job in Salesforce. If the client later begins another Staff Approved Job and is not enrolled in Next Steps, the SG Leader will create a new Staff-Approved Job. To learn how to create a Staff-Approved Job, click here. Secure Sustainable Housing Outcome Area

1. Evidence of Maintaining Residential Spaces and Common Areas in S&G: Clients start with full points. Each time a CLA or CLL records a Community Living Infraction with the Incident Type "Failure to Maintain Residential/Common Area" after the client enters Sustain and Grow, the client will lose points.

2. Following a Weekly Budget and Growing Savings: SG Leaders update the status to complete each month for the related the action item under the "Following a Weekly Budget and Growing Savings" Goal. To learn how to update the status of an Action Item, click here.

3. Reach Savings Goal of $3,000: SG Leaders update the status of the "Reaching Savings Goal of $3,000 Goal" in the Sustain and Grow Case Plan when it is completed by a client. To learn how to update the status of a Goal, click here.

4. Lease: SG Leaders update the status of the "Obtain a Lease" Goal in the Sustain and Grow Case Plan when it is completed by a client. To learn how to update the status of a Goal, click here.

When the SG Leader performs one of these Outcome Activities, both the individual Outcome area donut chart and the Outcome listview with Points will update automatically.
