Recording General Incidents

To ensure our clients' and staff members' safety on campus, we need to track incidents that occur for documentation and to ensure similar issues don't occur in the future.

This article describes how to document an incident on a client's profile. To document an Engagement Strike, click here. To document a Community Living Infraction, click here.

5. Enter a brief Subject to title the Infraction.

6. Type the Incident Description, ensuring you include the who, what, where, when, why, and how of the Incident.

If you find the incident itself or a client's behavior to be a risk, please complete the following steps.

7. Select the Risk Category.

8. Select the Indicator that prompted you to mark this as a Risk.

9. Select your Level of Concern.

10. Enter your Next Steps.

11. If this is a Mental/Behavioral Health risk, select Create Risk Assessment.

12. If this is a Mental/Behavioral Health risk, click Launch Risk Assessment.

13. Once the Assessment page has opened, click to return to the New Case: General Incident Tab.

14. Click Save.

15. If you need to record a Risk Assessment, return to the Assessment tab and complete the Risk Assessment.

The General Incident has been created and can be found in the Incidents section on the Client's Contact page.
