Helping a Mother or Guardian Fill Out the Children's Intake Form
Updated onArticleWhen a mother or guardian has been accepted into Atlanta Mission, we need them to fill out the Children's Intake form once they complete the Adult Intake form in order to collect information important for Children's Services case managers as well as to have them sign Releases of Information, photo waivers, and other agreements, etc. This article describes how to help a mother or guardian fill out the Children's Intake Form when they first arrive at Restoration House.
