Navigating a Client's Contact Page as a CLA

From the Client's Contact Page, CLAs can see details surrounding a client's demographics, health, faith, contact information, faith, and more. CLAs can also perform Risk Assessments, view and update Bed Assignments, Health Screenings, add Files, View and Update their medications, Cold Weather Screening, and Record drug screenings.

This article describes how to navigate the Contact Page as a CLA.

Highlights Panel

1. Client's Name.

2. Client's Photo: Intake Specialist uploads photo upon acceptance to Atlanta Mission.

3. Watchlist Icon: If a client has been found to be a risk to themselves or others within the last 2 weeks.

4. Age: Updated automatically based on their birthdate.

5. Phone: Their current phone number. If you click it, you can call them through RingCentral.

6. Email: Their current email. If you click it, Outlook should load and you can email them.

7. Last Infraction Notice #: The last Notice # for a Community Living Infraction. If they are approaching 6, a coaching conversation is warranted.

8. Site: The current campus site a client is living at.

9. Current Dorm: The current dorm a client is living in.

10. Current Bed: The current bed a client is assigned to.

11. Last Engagement Strike Notice #: The last Notice # for Engagement Strikes. If they are approaching 6, a coaching conversation is warranted.

12. Quick Action Buttons.

Bed Assignments

You can view all active and past Program Engagements for a client.

13. Bed Assignment Name: Always describes Bed Name - Date Assigned: Client Name.

14. Site: The site the client is currently living at.

15. Bed Assignment Date: The date the ded was Assigned.

16. Bed Discharge Date: The date the client was discharged from their bed.

17. Exit Reason: Shows the reason a bed was discharged.

Medication

18. Medication Name.

19. Dosage.

20. Frequency: How often the medication should be taken.

21. Date of Last Dose: If you are logging each time the medication is given in the system, this will show the last time a dose was given to the client. Doses will work in Phase 2.

22. Status: Is the medication Active or Disposed.

23. New: Add a new active medication to the Client's profile.

Health Screenings

24. Health Screening Name: Always describes Health Screening Type - Date Performed: Client Name.

25. Site: The Campus location the Health Screening was performed at.

26. Date: The date the Health Screening was performed.

27. Actions Taken: Any follow-up actions taken if a client has been coughing consistently.

28. Record Type: The type of Health Screening Performed (Intake TB, General Health Screening, or COVID Tests).

29. New: Perform a new Health Screening.

Drug Screens

30. Drug Name: Always describes Drug Screen Type - Date Performed: Client Name.

31. Date: The date the Drug Screen was performed.

32. Result: The Drug Screen result.

33. Test Type: The type of Drug Screen Performed (Alcohol, 12 Panel, 14 Panel, Kratom, or Nicotine).

35. New Drug Test(s): Record a new Drug Screen.

Files

These are specifically General Files accessible to all services staff. Clinical Files are coming soon.

36. Individual Files: Clickable to view.

37. View All: View all of the client's files if you don't see what you need in the preview.

38. Add Files: Add additional files to the client's general files.

Details

39. You can view and edit  a client's details surrounding demographics, contact information, health, legal info, faith, clothing, etc.

40. Click More.

You can access Client Notes, Assessments, and other Related Items from More.

Client Notes

Currently experiencing bugs. Should be fixed soon.

41. Filter: You can filter what Client Notes you see based on dates.

42. Client Note Previews: You can preview each note without the need to click in. You'll only see the information you have access to.

43. Print: You can print all notes from here.

44: New Note: Currently has a bug, do not use. In future, you'll be able to create a new note right from the client's contact page.

Assessments

45. Record Type: Controls which Assessment you view on the chart below. Only works for scored assessments (DLA and VTA).

46. Assessment Chart: Shows progress over time with scored assessments.

47. Start Date and End Date: Filters the time shown on the Assessment Chart.

48. Scroll down below the Graph.

49. Assessment Name: Clickable and leads to the individual Assessment Page.

50. Program Engagement: The Program Engagement the Assessment took place in.

51. Assessment Completed Date: The date the Assessment was completed.

52. Total Score: If the Assessment is a scored assessment, the total Score will appear here. (DLA and VTA).

53. New Assessment: Takes you to create a new Assessment.

54. Children: View a client's children.

55. Intakes: View all of a client's previous screenings and intakes including Cold Weather.

56. Service Deliveries: View all service deliveries for a client for all time.

Right Sidebar

57. Client Alerts: View all current alerts for a client. Soon you'll be able to create a new client alert from here as well. This section will show whether or not they are banned, in a bed, have an allergy, are in quarantine, have completed intake, have an Expired TB test, or are deceased.

58. Incidents: View all incidents including Community Living Infractions, Engagement Strikes, General Incidents, COATS, @Work Write-Ups, and DFCS Cases. You are also able to create new Incidents from this section.

62. Children: All children that have stayed with the mother or guardian at Atlanta Mission.
