Exit Interview Automation

There is a new field called "Sync Referral".  

Default behavior is to link any referrals (not already linked to an Exit Interview)  to the New Exit Interview on creation.

Recheck this box to force the Default behavior of linking referrals to the existing Exit Interview.

Referral Type Clean-up

In order to help clean-up Referral Reasons a field called Referral Type has been added as a classifying field.  So instead of displaying every option possible, we tried to limit to 6 types:

  • Educational & Training
  • Community & Social Services
  • Prevention & Wellness
  • Housing & Emergency Shelter
  • Professional Services
  • Behavioral Health Services

Referral Start / End Dates Relabeled + Page Layout

"Date Client Started / Appointment" is relabeled to "Start Date / First Appointment"

"Referral End Date" is relabeled to "End Date / Last Appointment"

Additionally the layout of the page has been modified slightly.

Bed Availability new values

Instead of Bed Availability being Available or Unavailable, we've added some further clarifying values.  Beds are now considered: Available, Occupied, or Out of Service.

Available - there are no client's in this bed and the bed is operable

Occupied - a client is currently assigned to this bed and the bed is operable

Out of Service - this bed is currently unoccupied but has been deemed in operable and in need of repairs.

Bed Status new value

Instead of Bed Status being blank, Permanently Offline, or Temporarily Unusable, we've added some further clarifying values. Bed Status now include: Online, Permanently Offline, or Temporarily Offline.

Online - Bed is operable

Temporarily Offline - Bed should not be assigned to a client either due to repairs or other reasons deemed necessary (i.e. renovations, etc)

Permanently Offline - Bed has been removed and should no longer be assigned clients (i.e. Top bunk removed from room and should no longer be counted in capacity numbers.)

new Client Phase fields + Automation

  • Visitor/Guest - new field for future automation (idea being default value until a client has engaged so many times with us.
  • Choose Help - current default
  • Pending Placement - automated after a client does a Needs Assessment; Ambassador is responsible to update phase to Pending Orientation or Transferred to Social Worker.
  • Transferred to Social Worker - when a client needs to work with a social worker to either be referred out or to address items prior to being moved to the Pending Orientation list.
  • Pending Orientation - when a client is waiting to move to Make Progress
  • Make Progress Orientation - automated when a client is assigned an Orientation Bed (will be deactivated in future release)
  • Make Progress - manual phase change when Make Progress Leader updates or automated when client is assigned a Residential Bed and previous phase is "Pending Orientation"
  • Sustain & Grow - manual phase or automated after Exit Interview "Exit Reason" equals "Completed Outcomes".
  • Referred Externally - manual phase or automated after Exit Interview "Exit Reason" equals "External Referral".
  • FH - No Team - manual phase when a client is a resident of Fuqua Hall but is not assigned a team.
  • Withdrawn - manual phase or automated from a lack of client

Automation from Lack of Client Activity

When a client hasn't checked in within the last 30 days and has no active bed assignment they will be automatically updated to Withdrawn.

Automation from Exit Interview

When a client is checked out of bed and an exit interview is completed the phase will be updated to either "Completed Program", "Externally Referred", or "Withdrawn".
